Public & Medical Records Requests
Public records and medical records requests may be submitted online, by email, or in person. Please do not include personal information beyond what is required to process your request, such as a date of birth or Social Security number.
- Records containing medical information are subject to HIPAA requirements.
- Additional documentation may be required to process your request.
- Billing statementsfor ambulance transport provided by Gig Harbor Fire & Medic One (PCFD #5) can be requested on our Transport Billing Questions page.

Records are available for inspection Monday through Thursday, 8:30 a.m. to 4:30 p.m. Fridays are available by appointment only. District administrative offices are closed on recognized holidays.
