Older Adult Fall Prevention & Home Safety

Falls are the leading cause of injury-related hospitalization for older adults in Washington State. Many falls for adults age 65 and older are caused by gradual health changes due to aging. Conditions such as chronic illnesses, daily medication use, and changes in muscle strength, balance, vision and walking ability cause falls. By learning to manage these health factors you can learn to stay active and independent for life.

File of Life 

File of Life is a non-profit program that helps first responders access your critical medical information during an emergency, including allergies, medications, and emergency contacts, when you may not be able to communicate that yourself. It's a simple, low-cost way to make sure paramedics have what they need right away.

Portable Orders for Life Sustaining Treatment (POLST) 

A POLST form is a portable medical order that summarizes a person's wishes for end-of-life care, such as resuscitation preferences, medical interventions, and artificial feeding. It's designed to travel with the patient between healthcare settings, so their care wishes are clearly communicated no matter where they receive treatment.
