Public Records & Medical Records Requests

Requests for records (public or medical) must be submitted in writing. 

To submit your request online, please complete the following online form. Do not include personal information outside of what is required on this form (such as DOB or SSN).

  • Any records that contain medical information will fall under HIPAA regulations.
  • We will follow up to obtain further documentation outside the online contact form.
