Public Records & Medical Records Requests
To submit your request online, please complete the following online form. Do not include personal information outside of what is required on this form (such as DOB or SSN).
- Any records that contain medical information will fall under HIPAA regulations.
- We will follow up to obtain further documentation outside the online contact form.
- Billing statements for ambulance transport provided by Gig Harbor Fire & Medic One (PCFD #5) can be requested by clicking here.

- Request for Public Records Form
- Requests for any records that include medical information will also require completing an Authorization to Use and Disclose PHI Form. Proof of identification must be submitted with this form.
