Public Records & Medical Records Requests
Requests for records (public or medical) must be submitted in writing.
To submit your request online, please complete the following online form. Do not include personal information outside of what is required on this form (such as DOB or SSN).
- Any records that contain medical information will fall under HIPPA regulations.
- We will follow up to obtain further documentation outside the online contact form.
To submit your request to our office by email or in person, please download the following form(s):
- Request for Public Records Form
- Requests for any records that include medical information will also require completing an Authorization to Use and Disclose PHI Form. Proof of identification must be submitted with this form.
